HOW DO I BECOME A PATIENT?
You can reach InSynch PT by email, phone, or sending a message through the contact page. We aim to schedule you as soon as possible with a 75-minute initial visit.
Once you are scheduled for your initial appointment, please be sure to download and complete the forms below prior to your appointment.
FIRST VISIT FORMS
DO YOU TAKE MY INSURANCE?
InSynch PT is a contracted provider with select insurance companies, including Regence, Cigna, and First Choice Health. This also allows us to submit claims for some Anthem plans, Blue Cross Blue Shield plans, and federal employee plans. We are unable to submit claims to Molina and United Healthcare.
DO YOU ACCEPT MEDICARE?
InSynch PT is not a provider for any government-funded programs such as Medicare or workmen’s compensation. Medicare patients can only be seen if all physical therapy benefits have been exhausted for the current year and Physical Therapy is no longer a “covered service.”

- Suitable clothing—shorts or exercise tights, please—no jeans!
